Non-Therapy Documentation Services

In addition to psychotherapy services, I may provide written documentation upon request when it is clinically appropriate and within the scope of my role as a Licensed Mental Health Counselor (LMHC) in Florida. Examples include Emotional Support Animal (ESA) letters, workplace or school accommodation forms, disability verification, FMLA paperwork, treatment summaries, and other third-party documentation.

Eligibility

Documentation is provided only for current clients with an established therapeutic relationship. I do not offer documentation-only services and do not provide letters or forms for individuals who are not active clients.

Clinical Discretion

All documentation requests are reviewed on a case-by-case basis. Completion of documentation is not guaranteed and must align with clinical judgment, ethical standards, and applicable state and federal laws. I reserve the right to decline any request that is not clinically appropriate or falls outside my scope of practice.

Fees (Flat Rate)

Documentation requests require time outside of therapy sessions and are billed separately at the following flat rates:

• Standard documentation letter or form: $75
  (e.g., brief accommodation or verification letters)

• Emotional Support Animal (ESA) letter: $150
  (includes clinical evaluation related to the request and letter preparation)

• Detailed letters or multi-page forms: $150
  (e.g., FMLA paperwork, detailed accommodation requests, treatment summaries)

• Court-related letters (non-forensic): $200
  (does not include court testimony or forensic evaluation)

All documentation fees must be paid in full prior to release. Insurance does not reimburse for documentation services.

Processing Time

Please allow 7–14 business days for completion of documentation requests. Expedited requests may not be available.

• Documentation reflects clinical opinion based on available information at the time it is written

• Approval by employers, housing providers, courts, or other entities is not guaranteed

• Documentation may be subject to verification by the requesting organization

Emotional Support Animal (ESA) Letters

ESA letters are provided only when clinically appropriate and in compliance with Florida Statute 760.27 and the Fair Housing Act. ESA letters are issued for housing accommodations only and do not apply to public access or air travel.
